## Three audio sources, one code path

1. **In-person consult** — room microphone captures both clinician and patient voices
2. **Video consult** — browser `getUserMedia` captures both voices through the laptop speakers
3. **Phone-on-speaker consult** — clinician’s mobile on speaker, room mic captures both voices acoustically

All three flow through `/api/transcribe` → a 4-layer Vercel fallback (Deepgram nova-3-medical → OpenAI gpt-4o-mini-transcribe → OpenAI whisper-1 → Groq whisper-large-v3-turbo) → Sydney Supabase. No CPaaS. No Vonage. No Twilio.

## Data residency

- Transcripts and clinical notes are stored in an **Australian-hosted Supabase database** (Sydney region, `ap-southeast-2`)
- **Audio is processed in-memory and never written to disk**
- All AI providers are contractually prohibited from training on patient data
